With Cleveland Clinic My Chart, you have access to your electronic medical records (EMR) right at your fingertips, by logging into the secure online patient portal.  Manage your healthcare online – at your convenience.

  • Enter your MyChart login
  • Enter your Password
  • Click ‘Sign In’

Sign Up for an Account

If you are a new user, begin by clicking ‘Set Up Your Account’ on the homepage.

  • Click ‘I already have an Activation Code’
  • Enter your activation code, Last 4 digits of your SSN and Date of Birth
  • Finally, Click ‘Next’ to proceed with Step 2
Do You Need an Activation Code?

First of all, you will need a one-time activation code in order to create a CCF My Chart Login to the patient portal.

To request an Activation Code, click ‘I Need an Activation Code’ during setup or submit your request directly here.

What can I do with a CCF MyChart Account ?

The online management tool provides you with a wealth of information regarding your health.  This not only includes; allowing you to view your medical test results, physician notes, current prescription medications.

Also, the patient portal provides access to preventive care recommendations & immunizations, electronic copies of you medical records and more.

MyImages

The MyImages website provides patients with the ability to view and request copies of their radiology images.

Even more helpful is that a significant portion of the radiology images generated at the hospital are available in the Image Library.

MyConsult – Online Medical Second Opinion

CCF-MyAccountIn addition to managing your health through CCF MyChart, the clinic makes it just as easy to pay your medical expenses with MyAccount Online Bill Pay.
