To provide patients with an easy and secure way to manage their health care online, many of the top doctors, health insurance companies, and other providers utilize MyChart. This includes DeanCare, Kettering My Chart, UF Library, Nicollet, Cleveland Clinic, MyUW Health.

How to Access Your Login

Your account access makes it easy to put your medical records instantly at your fingertips. Be sure to have your login credentials and instructions on hand as provided by your doctor or provider to access your medical history online.

  • Visit your provider’s patient portal
  • Enter your username and password
  • Click the ‘Sign In’ button

Are you a New User?  Learn How to Sign Up for the Patient Portal

  • To get started, visit the patient portal for your medical provider
  • Click on the ‘Sign Up Now’ button
  • You will be asked to enter your activation code, as shown on your enrollment paperwork

Followed by a series of questions to confirm your identity such as; Date of Birth, Postal Code and Home Phone number.

If you do not have a code, you must obtain a one-time activation code from your doctor’s office.

Features of My Chart Patient Portal by Epic Systems

The Epic EMR system is one of the most widely used patient portals to manage electronic health records (EHR) or electronic medical records (EMR).

It provides a service similar to competitors, such as NextMD patient portal, AthenaHealth, RelayHealth, MyHealthOnline, and Follow My Health.

Online access is an easy and secure way to view and share your electronic health records online. With My Chart, patients can conveniently download medical records and communicate with their doctor via secure messaging. In addition, you can find doctors in-network under your new health insurance plan, schedule doctors’ appointment, view medications, request prescription refills, and so much more.

View your Electronic Health Care Records (EHR) with the Mobile App


If you would rather access your medical records from your smartphone, you can utilize the mobile app to enter the patient portal.

From there, you can access all the same features as the online patient portal.

You can view your electronic medical records, schedule doctors’ appointments, request prescription refills, find care providers, and more.  All from the convenience of your smartphone

The My Chart app is available on Apple iPhone and Android.  Once you download from iTunes or the Google store, enter your login credentials.

Virtual Care with Telemedicine: A Partnership with Epic Systems EMR and the Mayo Clinic

Epic Systems recently announced a new partnership with the Mayo Clinic Care Plans;  Virtual Care.


For patients being discharged from the hospital or managing a chronic health condition, the new telehealth feature shows a personalized care program to follow.

Both patients and caregivers can benefit from the newest telemedicine feature, Virtual Care.

The telehealth tool provides step-by-step guidance and support to help manage a patient’s ongoing care.

  1. If I switch to NextMD, will they transfer my records so I can see past medical history from them online and also see my current health records from my NextMD?

  2. I have an account for my father based on the fact that I am his DPOA. He suffers from progressive MS and dementia. His health care providers have repeatedly ignored my requests to be his proxy and consistently revoked my access to the account. His health is suffering because of it. The DPOA is on file in multiple departments at the hospital. It clearly lists my right to access all of my father’s information without restrictions.
    It is illegal to prevent me from helping my dad and shame on that.

    1. go to medical records and ask for his entire profile and they will give them to you. if he was like me he will have many pages to view, mine was 10 pages and they had to put it on a CD takes a couple of days to get permission from the dr’s …is someone asking for cause of death from the insurance…?

  10. I got a login page from my covid test at CVS. I previously had the app on my phone from years ago, so have an account already, but can not log in. The user name is a mystery. I am trying to find a contact phone number for regaining access to my test results.
