How do I sign up for a MyChart login?

You must be a UWHealth MyChart patient to sign up. Call the MyChart Patient Support Line to request an activation code, or visit your nearest clinic to get one. After receiving the activation code, you can activate your account online by visiting the UWHealth MyChart site and following the instructions.

Can I view my medical test results through UW Health Home Access?
